I have seen and have done all three myself for different belts so that is a good question. Something else is that it could vary within lineage of instructor belt by belt. In other words, they give away the blue by surprise, have a formal roll for purple, and formal test for brown or black etc.
Having gone through to black belt in TKD, I recall all the crap associated with formalized testing. The Korean masters would fly in, you had to know X-number of forms, do a certain breaking technique, set one-step/three-step sparring requirements, free sparring, and even a language test. I wasn't at a McDojo, so sometimes people didn't pass ( even if they paid ), but my instructor would discourage anyone that he thought wasn't ready from testing, so it wasn't a money machine.

All my belts have been scheduled promotions except for the purple belt, which was thown across my shoulder during a no-gi class. I've never tested for a belt, though.

actual testing is the only way to go. how else do you know how much info the person knows with out that?
can they actually show you moves or can they just scramble & hang on for 1/2 an hour

Purple was the only one that was a "scheduled" promotion. Every other belt was a suprise. When I was promoted to brown belt, my instructor pretended like he was going to demonstrate a clock choke to the class and use me as a dummy. When he was explaining the move, he pulled the belt out of his gi and slapped it on me. I thought it was pretty cool.

If the instructor teaches all his classes AND watch his students spar and drill AND he spar his students and have half a brain AND he is keeping track of the students attendance which allows him to see that the students is putting in the time then the instructor can determine the skill level of his students without a test
